Zócalo Health

Zócalo Health is a community based healthcare organization that connects Medi Cal members and families to the care and support they need to stay healthy. We work directly in the neighborhoods we serve, partnering with local organizations, schools, and community leaders to reach individuals who may need extra help navigating the healthcare system.

Our team provides Enhanced Care Management, Community Health Worker support, and connections to medical, behavioral health, and social services. This can include help scheduling appointments, finding a primary care provider, accessing food or housing resources, and coordinating care for members with complex health needs.

At Zócalo Health, we believe healthcare should be personal, culturally responsive, and easy to understand. We meet people where they are, build trust, and walk alongside them to make sure they feel supported every step of the way.
